Pregnancy mods for the Sims 4 give players a lot of customization options to tailor their pregnancy experience, and we've rounded up some of the best.

2. Pregnancy Preferences & Reactions & Impact

Does your Sim want children right now, is ok with anything, or does not want children? What about their partner? And how will all that play out if they don’t share the same wish? Do your Sim’s kids want siblings? Do their parents want grandchildren? How will everyone react to a new family member?

All Sims age young adult – adult are automatically given pregnancy preferences (wants children, neutral, does not want children). Sims who hate children always auto-get ‘dosn’t want children’ – and the opposite is true for Sims who are family-oriented.

Preferences can be changed at any time, for the played Sim in the Pregnancy & Family menu, for any other Sim, by clicking on them and going to “Actions” then “RPO Special Actions”.

Preferences can be given manually to teens. If the teen module is installed and a teen who has no assigned preference gets pregnant they are automatically given the ‘Doesn’t Want Children” preference.

3. Fertility and Protection – a Sims 4 mod

Individual or Global pregnancy risk, birth control, surgeries, social interactions!

WooHoo has a chance of causing pregnancy. The chance is a number based on the fertility of each Sim involved and some minor other factors.

Protection (woohoo protection items & birth control) can be used. These methods have a 5% failure rate. If they fail, the risk rate as mentioned above will be applied.

4. WooHoo Transmissible Diseases in Sims 4

Diseases, accusations, acceptance or rejection, etc. NPC Sims in the world will have a chance to get diseases randomly upon first appearing or upon installing the mod. 

You can manually give diseases through cheats (in the “Relationship Wellness” menu, then “Sim Status”, then “WTD Cheats”)  

Diseases are either fully asymptomatic, or with gradual symptoms.

In which Teen Sims can freak out about being pregnant, make their parents freak out, get support or get kicked out…

Teens do not get pregnancy wishes by default; you can assign them manually. BUT If a Teen who doesn’t have a wish finds out they’re pregnant, it will by default assign the ‘Does Not Want a Pregnancy’ wish and reaction to both the pregnant Sim and the father.

Paternity testing, pregnancy preference effects, etc etc, all works with teens the same as with adults.

When parents are told about a teen pregnancy, this happens for them to decide how to react.

7. Temporary Separations / Relationship Breaks 

Have your Sims take a relationship break and try to figure things out before fully breaking up or reconciling.

- ‘Separated’ status in between full on broken-up / divorced and being in a couple

Separated Sims are keeping track of what happens during the separation; they each have a feeling about each other that will improve or worsen depending on what happens during the separation, guiding whether they should eventually make up or break up.

One of the Sims can decide to (or be asked to) go stay somewhere else temporarily, or to outright move out, to make the separation easier, as there may be negative sentiments in the air that will participate in wrecking the relationship further…

Once the “Love Languages” module of this modis out, they will be able to focus on learning each other’s love language and try to communicate with each other better.

They can, once ready decide to try to patch up / stay together, or decide to simply end the relationship permanently.

8. Cheating (Infidelity) Expansion & Overhaul

You're sims really get to experience the rollercoaster of emotions when it comes to cheating. You get to decide why your sim cheated and how their partner will react! Cheating as it is, is boring. Now sims will have to face the effects of their affairs and indescresions! From reasons to reactions and everything you need this truly is the most detailed cheating mod there is!

A module that changes the sentiment behavior for cheating,tier-ing it depending on the gravity of the fault, and offers added features and possibilities revolving around cheating. See different reactions depending on whether a Sim only flirted with another, kissed, or woohoo’d. Blame, forgive, lie about having ‘only flirted nothing more”, confess to cheating, tell a Sim your Sim slept with their partner… and more!

It is, in part, completely new stuff, and in part a big re-design of my old Cheating Module from Contextual Social Interactions (that Cheating Module was the very first mod I ever made). It will work hand in hand with another module, the Romantic Trust System.

9. Artificial inseminations & Surrogacy

A mod that offers the possibility for Sims to get artificial inseminations, become surrogate mothers or have another Sim carry a baby for them.

1. ARTIFICIAL INSEMINATIONS

To get artificially inseminated, your sim must choose ‘Attempt Artificial Inseination’, under “Pregnancy and Family” and then “Alternative Pregnancy”.

Pregnancy will go on normally. Once of teen age, children can be told about how they were conceived.

2. HIRING NPC TO CARRY PLAYED SIM’S BABY

“Have a Baby through a Surrogate” is available in “Family and Pregnancy” and then “Alternative Pregnancy”.
Your Sim will call the Agency of Surrogate Mothers and then can choose amongst the available surrogate mothers.

For the artificial insemination of the surrogate:
– If your Sim can impregnate, they will father the child.
– If your Sim cannot impregnate but they have a partner who can, the partner will father the child.
– If your Sim does not have a partner who can impregnate and cannot impregnate, the lab will pick a sample to get the surrogate’s pregnancy started.

3. ACTIVE SIM CARRYING BABY FOR OTHER SIM

Your Sim can decide to “Carry a Baby for Other” (under “Pregnancy and Family” and then “Alternative Pregnancies”), which will lead to a phone call to the Agency of Surrogate Mothers and a visit to the lab for insemination.

10. Adoption Expansion & Overhaul in Sims 4

Lets your Sims put their children up for adoption, via agencies or into the household of friends or family, after agreeing or fighting. Also, offers a different way to adopt children. Instead of just calling a phone number, leaving and coming back with a child, Sims have to satisfy the criteria of adoption agents. Sims are able to look for their biological parents.

Because adopting a child shouldn't be like choosing candy in a store.. finally we have realistic options to adding a new family member in our home!

11. Custody & Temporary Separations in Sims 4

Do you ever wish there was more depth to the divorce process in The Sims 4? Would you rather see your sims take each other to court to fight over assets and custody of their kids? Then this is probably the mod for you!

– Splitting of estate / funds, in case of permanent separations

– Amicable or non-amicable permanent separation

Break-Ups and divorces can be attempted amicably. If one of the parties involved is against the divorce, it may still go sour, but there is a good chance that the amicable approach makes things easier for both sims involved. They can this way stay in good terms and have special interactions.

12. Termination of Pregnancy in Sims 4

Termination of Pregnancy, and Reactions to it for pregnant Sims and their partners.

In this (very optional) add-on, Sims can terminate pregnancies, accompany spouses to pregnancy termination, and experience the myriad of emotions that such a difficult decision can bring – differently according to pregnancy preferences as chosen in Mod 1 of course.

Reactions depends on pregnancy wish and on a bit of chance.
Sims who want children will be immediately sad if they have to terminate a pregnancy.
Other sims will be dazed at first, unable to figure out what they feel. A bit later, clearer emotions will come to the surface. They can feel guilt, relief, sadness, anger, and can go through several of those as they remember the event over the next few days, with several possible moodlets. Some Sims will eventually feel relief, while others may be stuck on guilt alongside that.

13. Pregnancy Side Effects in Sims 4

An add-on that brings mood swings, cravings and back pain to Sim’s pregnancies, among other little things!

Pregnant Sims will occasionally get mood swings, cravings, and back pain. When this happens, new interactions unlock (Complain about back pain – partners can ‘Offer massage for Pregnancy Back Pain‘ -, Apologize for Crankiness, Complain about mood swings, Ask for Craved Food, Go get craved Food). The ‘mood swing’ moodlets increase the intensity of sad and angry moodlets overall. NOTE: This can be turned off for individual SIms, under ‘Actions’ then ‘WooHoo Wellness Mod’

14. Miscarriages & Pregnancy Complications – a Sims 4 Mod

TW Pregnancy Loss. This add-on makes it so that Sims might have miscarriages and ectopic pregnancies. Either a global or a different risk level, various factors taken into account. Statistical and event-triggered.

Lost pregnancies can optionally be added to family tree, create an urn in the Sim’s inventory which enables them to name the unborn child and mourn them.

Features:

• Chances of miscarriage (statistical and event-triggered) or ectopic pregnancies.
• Sims who can be pregnant each have a certain risk of issues during pregnancy, to globally translate variations in health and biological configuration (diabetes, unusual uterus shapes, etc etc).
• If a miscarriage happens, a Sim will remember it for some days and for the next pregnancies. They will also have an urn to represent the child, can name it and mourn, and integrate them to the family tree.
• Reactions vary based on Sim’s preferences regarding having children. There are interactions to tell about moscarriages and tell about times where the pregnancy was saved as well.

15. Pregnancy Tweaks & Special Pregnancies in Sims 4

Pregnancy Tweaks & Special Pregnancies (RPO, Mod 14) - No Walkstyle, Cryptic pregnancy, and more - Sims 4

Settings and tweaks for pregnancy, such as disabling the pregnancy walkstyle, the idle animation, etc, enabling cryptic pregnancies, etc.

There is a 5% base chance of cryptic pregnancies happening if you have that setting switched on. You can turn it off in global settings, still under “actions” as always.

If you want to CHEAT one of these to happen, you can. Just make sure you have “debug and special actions” enabled in settings, then go to the main RPO menu -> Gen. management -> Gen. debug -> add stat, and add to the relevant one. (Of course, do that only if you already know your Sim is pregnant, and your Sim isn’t showing yet.)
